Ultrasound GuidedInjections for Knee Arthritis

Most patients suffering from knee pain related to osteoarthritis can find great relief with synthetic joint fluid injections. If you have been told you may need a knee replacement, and are not ready for the procedure, synthetic joint fluid injections can be used to help relieve pain as part of our knee-replacement deferral programs.  One of the most successful non-operative treatment options that is available to address this problem is known as “viscosupplementation” or the injection of synthetic hyaluronic acid into the knee joint.  This artificial joint fluid is often derived from the rooster comb, thus it has been nicknamed the “rooster shot” for the knees. This fluid acts as a cushion between the two bones of the knee and can result in less stiffness and pain, as well as greater mobility.

Platelet Rich Plasma (PRP) Injections

PRP is a treatment that uses only the patient’s own blood to stimulate a healing response for arthritis.  The process starts by drawing the patient’s own blood and then placing it into a centrifuge so that the platelets can be separated from the rest of the blood. Once the platelets have been separated, they are then injected into the injured area to stimulate a healing response and reduce the inflammation and pain associated with arthritis.

A recent study showed that 73% of patients (ages 30-70) with Stage 1 and Stage 2 osteoarthritis of the knee had reduced pain levels, increased functionality of the knee, and no progression of these early stages of arthritis after receiving the PRP injections.

Partial Knee Replacement

In many cases, damage as a result of arthritis may only affect one area (compartment) of the knee joint. For patients with damage to only one compartment of the knee a Partial Knee Replacement (also known as “Unicompartmental Knee Replacement”) may be considered.

Partial Knee Replacement consists of 3 basic steps:

Preparing the bone. The surgeon will use special tools to remove damaged cartilage from the only one compartment of your knee while leaving the other compartment of the knee completely intact.

Positioning of the implants: The removed cartilage and bone is replaced with metal components that cap the ends of the trimmed compartment, thus recreating the surface of the joint. These metal parts may be cemented or “press-fit” into the bone.

Inserting the spacer: A medical-grade ultra high molecular weight polyethylene plastic spacer is inserted between the metal components to create a smooth gliding surface that replicates the natural motion of the knee.

Total Knee Replacement

In more severe cases, the arthritic process has caused the cartilage on the ends of the bones in the knee to wear off.   This is most common with osteoarthritis. The exposed bone, which has nerve endings, rubs against itself and causes pain and swelling. If conservative treatments such as anti-inflammatory medications, physical therapy, corticosteroid or artificial joint fluid injections, fail to relieve the pain in an arthritic knee, then Total Knee Replacement may be considered. Total Knee Replacement consists of 4 basic steps:

Preparation of the bone: The damaged cartilage surfaces at the ends of the femur and tibia are removed along with a small amount of underlying bone.

Positioning of the implants: The removed cartilage and bone is replaced with metal components that cap the ends of the trimmed compartment, thus recreating the surface of the joint. These metal parts may be cemented or “press-fit” into the bone.

Resurfacing of the patella: The under-surface of the patella (kneecap) is cut and resurfaced with a plastic button.

Inserting the spacer: A medical-grade ultra high molecular weight polyethylene plastic spacer is inserted between the metal components to create a smooth gliding surface that replicates the natural motion of the knee.
